چکیده

Pre-labour premature rupture of membranes (PPROM) is defined as rupture of membranes more than 1 hour prior to the onset of labour at <37 weeks gestation. PPROM occurs in approximately 3% of pregnancies and is responsible for a third of all preterm births.1 Once membranes are ruptured prolonging the pregnancy has no maternal physical advantage but fetal morbidity and mortality are improved daily at early gestations: 19% of those infants born <25 weeks develop cerebral palsy (CP) and 28% have severe motor disability.2 Those infants born extremely pre term (<28 weeks) cost the public sector £75835 (95% CI £27906–145508) per live birth3 not to mention the emotional cost to the family. To prolong gestation is therefore the suggested goal: however how and why might we delay birth in those at risk? PPROM is one scenario associated with preterm birth and here we discuss the causative mechanisms, sequelae, latency, strategies to prolong gestation (antibiotics) and consider the role of amniocentesis. We will also discuss novel therapies.

The Outcome of Amniocentesis at 14 Weeks of Gestation

Method: This was a retrospective study of the pregnancy outcome of 299 women who underwent invasive prenatal diagnosis by using amniocentesis at 14 weeks of gestation, amniocentesis at ≥15 weeks of gestation and chorionic villous sampling. We compared the pregnancy outcomes and the complications in the 3 groups by using the Pearson’s χ2 test or the Fisher’s exact test. Results: There was no sig...

MISOPROSTOL, (Low Dose for labour induction at term)

Labour induction is a cornerstone treatment for a broad range of conditions −some of which are common, for which a prompt delivery reduces maternal or neonatal morbidity and mortality. Examples of frequent conditions are postterm pregnancy, pregnancy induced hypertension, pre-eclampsia and eclampsia, premature rupture of membranes, intrauterine fetal growth retardation chorioamnionitis, fetal d...

WEST AFRICAN JOURNAL OF MEDICINE Spontaneous Pre-Labour Rupture of Membranes at Term: Immediate versus Delayed Induction of Labour La Rupture Pré-de la main-d’œuvre Spontanée de Membranes au Terme : Immédiat contre l’Induction Retardée de parti travailliste

BACKGROUND: Spontaneous pre-labour rupture of membranes (SPROM) at term is one of the most common complications of pregnancy. It is an important cause of perinatal morbidity and mortality, particularly because it is associated with a latency period from membrane rupture to delivery. OBJECTIVE: To compare the outcome of labour in women who had immediate induction of labour, with those who had de...

A Clinical Study on Expectant Management versus Induction of Labour in term Premature Rupture of Membranes (PROM)

Premature or Prelabour Rupture Of Membranes is classically defined as the rupture of integrity of the fetal membranes before the onset of labour and resulting in leakage of amniotic fluid [1-4]. Pre-labour rupture of membranes without spontaneous uterine contractions complicate approximately 10 % of all pregnancies, out of which 80 % occurs at term [5]. The management of PROM at term remains a ...
